Part of your first visit to our office will be to gather information that will be used to treat you more effectively.  For us to do this, we need your help to make sure that you have all of the items listed below with you for your first visit.

Also our job is to answer all the questions you have regarding your care so we ask you to write them down so they are not forgotten during our initial exam.

WHAT YOU NEED TO BRING

1. Please have a picture I.D. (drivers license or photo I.D. card) and your insurance card
2. New Patient Forms (available on the website) need to be completed in full.  This information will help us identify your main problem and your health history.  Please fill in all balnks.  If the question does not apply to you, put "NA" in the space.
3. Please sign the New Patient Privacy Notices
